CLEVELAND GOLF AWARDS UPDATE

CLEVELAND GOLF CANADA PLAYERS OF THE YEAR AWARDS
Presented to a player in each age group who plays in at least three MJT events in 2004; is a member in good standing; has the lowest stroke average in their division; and demonstrates good sportsmanship and conduct.
Please note: No application is necessary to qualify for Players of the Year Awards.

CLEVELAND GOLF CANADA SCHOLARSHIP AWARD
A $1,000 Scholarship to be awarded to one applicant who shows exemplary athletic, educational and personal achievements during the 2004 season and who meets the eligibility requirements, including:
? Applicants must apply in writing to MJT, P.O. Box 18083, Tsawwassen, BC, Canada V4L 2B0 not later than Friday, September 12, 2004.
? Applications for consideration must be submitted with a complete Cleveland Golf Canada Scholarship Application Form (Click Here) along with the following items and information:
? High School or College/University Transcript showing marks for the 2003/04 school year.
? Summary of personal achievements including community involvement, leadership roles, extra-curricular activities, work and volunteer efforts.
? A good MJT stroke average and/or top finishes in MJT events.
? A 300-500 word essay: How were you first introduced to the game of golf and what effect or impact has this sport had in your life?
? Applications open to current MJT members who are Canadian residents and who have played in at least two MJT events in 2004.
? Applicants must be enrolled in High School, College, University or other post-secondary education program in Canada or the United States for the 2004/05 school term.
? Applicants with demonstrated financial need may receive priority. Proof of household income may be required, such as statement of earnings from employers.
The Cleveland Golf Canada Scholarship will be paid directly to the educational institution of choice by the winner or to an R.E.S.P. account.
